Register for the March 28, 2026 Youth Division Rotating Partner Round Robin Tournament. Players in Grades 5-12 are eligible to play. (Younger players must be pre-approved.)

This is to certify that I, as parent/guardian with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward. We have discussed the risks of the activity and his/her responsibilities for adhering to the rules and regulations. These risks include, but are not limited to, injuries resulting from falls, collisions, and being hit by a ball or paddle. He/she understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to this release and agree to indemnify and hold harmless Yucaipa Pickleball, Inc., its Board of Directors, officers, and volunteers and the City of Yucaipa and its agents, officers, and volunteers from any and all liabilities incident to my minor child’s/ward’s involvement or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law for this and all future events. I further agree that Yucaipa Pickleball, Inc. may use any and all pictures of my child/ward in the normal course of events for its documentation or publicity of the event on social media or otherwise.
