Junior Membership (Under 18)

Our membership application procedure is in two parts.

Part 1: Please fill out the application and make sure all text fields are completed. The application will not send with blank fields.

Part 2: Payment options for membership.


Part 1 – Application

I hereby Pledge to live up to my responsibilities as a Yucaipa Pickleball, Inc. Junior Member by following this Player Code of Ethics.
I will show good sportsmanship to players, coaches, officials, opponents, and parents at every game and practice. I will not yell at my teammates, opponents, coaches, or referees. I will refrain from using foul language, obscene gestures, or other inappropriate behavior. I will learn the value of commitment by participating in as many practices and games as I can. I will encourage my teammates and praise good efforts. I will be honest, fair, and respectful to others always. I will aim to learn all I can from Pickleball.
With my signature, I as the Junior Member, acknowledge that I have read, understand, and accept the terms of, and I will do my best to fulfill the terms of this document. I understand that not upholding the terms of this code and agreement can result in removal from the Yucaipa Pickleball Club.

As the parent or guardian, I certify that my minor child is physically fit and is in a condition to play Pickleball.  I agree to assume all responsibility for all risk, damage, or injury that may occur to him/her as a participant. I understand there is a risk for COVID exposure in participating in this sport. I agree he/she will not participate if feeling ill, showing any signs of COVID or having tested positive within two weeks of participation. I release and discharge him/her, myself, my heirs, executors, and administrators, the organizers, coaches, the facility, its members, employees, and sponsors and all persons associated with Yucaipa Pickleball, Inc. activities from all claims, damages, rights of action present or future which may arise in connection with my child's participation in Yucaipa Pickleball, Inc. events.

Thank you in advance for your contribution to Yucaipa Pickleball, Inc. a non-profit 501(c)3u003c. Credit Card payments will receive a receipt for your donation to the email you provide during the payment process. If you can’t find your receipt, please check your spam folder.


Zelle

$25.00 to yucaipapickleball@gmail.com and please designate in your description as a “membership fee” to help us distinguish what type of payment you are making. Check with your Zelle account for confirmation for your payment.


Venmo

$25.00 to @Yucaipa-pickleball-1 and please designate in your description as a “membership fee” to help us distinguish what type of payment you are making. Check with your Venmo account for confirmation for your payment. Please be sure you have NOT selected the toggle “Turn on for purchases” for the payment. Otherwise Venmo charges us an additional fee. 


Credit Card

$27.50 by credit card. Your credit card payment include a fee of $2.50 for processing. Credit Card payments will receive a receipt from our payment processor to the email you provide during the payment process. If you can’t find your receipt, please check your spam folder.
